Faculty Peer Review Forms

Faculty peer evaluation is an essential part of the evaluation process. Such feedback helps identify the strengths of a faculty member, helps identify areas for improvement, and helps in making decisions to retain, to grant tenure, and/or to promote faculty. Please contribute to the process by writing an evaluation of this faculty member for each of the categories listed on the review form. When undertaking formal reviews of faculty performance and accomplishments, the VPAA recommends that you familiarize yourself with the review criteria set out in the Faculty Handbook.

To aid you in this task, a selection of tables with appropriate review criteria has been developed. You may access this collection of review criteria here: REVIEW CRITERIA.

In an effort facilitate a clear and accurate documentation of your assessment, you are asked to submit one of the following appropriate forms.  

Once you hit "submit" - the form will be sent to Brenda Meyer who will then add it to collection of data under consideration.  

For protection, each form is password protected. You received the password when Brenda first contact you. If, for some reason, you have misplaced this, please contact Brenda for the password.

We highly recommend reviewing the appropriate form and typing out your comments on a separate page and “copy and paste” your reflections into the form before hitting “submit”. This way you have the opportunity to carefully revise your judgments prior to the formal submission.
